CONNECTPlus · Health and Care Video Library

Reduce demand through patient information.

Trusts and ICBs use CONNECTPlus and the Health and Care Video Library to put information to work in three ways: education that stops a question becoming a call, data that stops a routine follow up becoming an unnecessary visit, and insight that stops a referral sitting on a waiting list.

What information already does

  • 87% completed a fully digital pathwayMid Cheshire Hospitals, against a typical rate of around 25% for digital health tools
  • 4× more patients seen per nurse hourVideo replacing face to face medication education, Penford et al, 2023
  • 40%+ fewer helpline callsReported outcome, NHS trust client
  • 18% lower non-elective admission costsTorbay and South Devon, combined digital approach, 2025
Independent evidence, not just our own case studies.

Three ways information reduces demand

Each flow of information takes pressure off a different part of the pathway.

For patients

Fewer calls, fewer DNAs

Clear, clinician-approved video answers the question before it becomes a phone call, a missed appointment or an unplanned visit.

40%+ fewer helpline calls

From patients

Shorter, fewer appointments

Symptom and PROM data captured at home lets routine follow ups move from face to face to a short call, or be safely skipped when nothing has changed.

Appointment length halved, Mid Cheshire 2026

About patients

Faster triage, safer discharge

Dashboards built from that data let clinicians prioritise the patients who need them most, and safely step down or discharge the ones who do not.

What this looks like for one pathway

An illustrative example from HCI’s own impact model, for a typical pathway with 2,000 patients on the books, 500 referrals a year and 2,000 consultations a year.

Backed by independent evidence

These are not only HCI’s own claims. Independent clinicians and researchers, publishing in peer-reviewed and conference literature, describe what happened when they used CONNECTPlus and the Health and Care Video Library.

An independent review in The Lancet Digital Health found app based interventions produced a significant benefit on 21 of 31 outcomes studied, across 78 systematic reviews and 496 trials covering 177,373 participants, with no industry funding.

Vasiloglou et al, The Lancet Digital Health, July 2026

In a 2023 rheumatology study, moving medication education from face to face to video nearly tripled how many patients a nurse could see in an hour, while cutting the cost per patient by two thirds.
