The article focuses on the issues relating to successfully delivering care closer to home. We absolutely need to do it and the big question is how do we make it happen? What we've tried to do is highlight some of the key issues that need to be addressed.

These are all challenges that we at HCI can help healthcare systems resolve. We are already reducing demand in the NHS using CONNECTPlus, supporting over 70 conditions and treatment pathways, from medication management in immunology, through heart failure, pulmonary rehab, preparation for surgery, and diabetic foot management in the community amongst many others.
